Please submit the following form, a highlighted plat map, a GPS printed map, or other suitable maps for fields to be inspected. Indicate the variety of the seed field and surrounding fields and major cross roads. If your field is not visible from the road, indicate this on the map and give necessary instructions to locate field. If directions to a field are not sufficient or clear for the inspector, this may be sufficient reason for not getting field inspection completed.
